Every Person Is Considered Individually
We do not make admission decisions based on a diagnosis alone. Before admission, we review the person's health and support needs, medications, mobility,personal-care needs, cognitive or behavioral support needs when applicable, dietary needs, routines, preferences, safety considerations, and other information necessary to determinewhether our home can safely and appropriately provide the needed services.
Individual Review
We start with a conversation and an individualized review of the person's needs.Admission depends on whether the person's needs can be safely and appropriately met withinthe services, staffing, competencies, equipment, physical environment, and other resourcesavailable at our home.
Service Planning
Services are based on assessment and an individualized service plan. When a resident's needschange, those changes are addressed through the appropriate assessment, service-planning,clinical, and care-coordination processes.
Waiver-Funded Services
Customized Living and 24-Hour Customized Living are available to eligible individuals through Minnesota waiver programs. Services require assessment, authorization, inclusion in the service plan, provider availability, and proper documentation.
